Interested in attending Camp Alphie? We can't wait to see you! Whether you're a cancer patient yourself or the sibling of one, you'll need to fill out these forms to attend the camp. When they're all sent in, get ready for six days of pure fun!

 

Remember, every form must be filled out completely to be considered to attend the camp! That means signatures, dates, and every piece of information required must be filled out for you to come to Camp Alphie. All forms are due June 15th!
